PATIENT SCHEDULING

A medical or dental office may book an appointment with us on behalf of a patient.

If you are a patient who wishes to use our services, please contact your dental/medical office or contact us via phone to discuss your needs.

Option 1.

SCHEDULE A DENTAL SEDATION APPOINTMENT ON BEHALF OF A PATIENT WITH DEPOSIT

Please click on the Dental Patients Needing Anesthesia option below.
Under the Service column choose the length of appointment needed between 1 HR and 6 HR.
Click Book Now. The cost to make any booking requires a $515 deposit and the remainder of the balance is due from the patient at the appointment.
Next, using the calendar choose the next available date and start time you would like.
Fill in the required patient information and click Continue.
Next, enter the Office, Doctor, Procedure, and Length and click Continue.
Please review the booking date/time and if it is correct then enter the credit card information and click Submit.
The anesthesia intake forms link will be sent immediately to the patients email to be completed.
Please ensure the patient knows to fill out those forms immediately in order to ensure a smooth process.

Option 2:

OFFICES WITH RESERVED SERVICE DAYS (NO CALENDAR SCHEDULING)

For offices who have already scheduled a reserved day with us.

Under Category, Choose Offices with Reserved Service Days
Choose the appointment duration by selecting the package and clicking Sign Up.
Enter the patient information and click Continue.
Enter the Office and Procedure information and click Continue.
Enter the credit card information and click Submit.
The anesthesia Intake Forms link will be sent immediately to the patients email to be completed. Please ensure the patient knows to fill out these forms immediately in order to ensure a smooth process.
